Family Practice/Primary Care Nurse Practitioner

Specialty accepted: Nurse Practitioner or Physician Assistant - Internal Medicine or Geriatric
Start date: As soon as credentialed - ongoing (6-month minimum, 1-year preferred)
Schedule: Monday-Friday 8am-5pm (8-hour guarantee)
Practice setting: Home Health
Patient load: 3-4 visits per day on average (up to 6 if close proximity)
Ages: Adult patients
Procedures required: Primary Care, Wellness Exams, Chronic Disease Management, Population Health, Kidney Care
Requirements: BLS, DEA, Active West Virginia License, Board Certified
Other: Must be comfortable with home health setting. Long-term opportunity with flexibility. Must be willing to supervise RN/LCSW support. Orientation is a 3-day paid trip to the national office in Brentwood, TN. Travel between visits is local and commutable (under 1 hour average). Mileage reimbursed at $0.44 per mile.
