SUMMARY DESCRIPTION OF PROGRAM

Heartland Bridges houses two programs, Bridges Transitional Living and Bridges Crisis Care. Transitional Living is a short-term transitional housing program enriched with supportive services designed to assist people with a diagnosed mental health or co-occurring condition while they transition to stability in the community. Crisis Care is a residential stabilization service designed for individuals who need a safe, secure environment for 3-5 days while stabilizing from a crisis before returning to the community where they live.

SUMMARY DESCRIPTION OF WORK

The Lead Case Manager serves as the program’s lead crisis stabilization specialist that provides intensive supervision, de-escalation, and case management services to all four of the facility’s Crisis Care participants and up to three Bridges Transitional Living clients. This role coordinates supportive services designed to meet the needs of the client while working with them to develop a plan for stabilization. Decision making, problem solving, and highly developed interpersonal skills are critical.

Compensation: between $21.79 and $25.98 per hour (wage is determined upon total years of relevant experience).
Work Schedule: 40 hours per week

MINIMUM QUALIFICATIONS

  • Bachelor’s Degree in human services field (including but not limited to, psychology, social work, mental health counseling, marriage and family therapy, or a related field)
  • Three years of experience in behavioral or mental health
  • Experience working with adults with serious mental illness or substance use preferred
  • Valid driver’s license and acceptable driving record

Essential Duties and Responsibilities

  • Responds to crisis referral calls, assesses individual by gathering information and provides appropriate and accurate application verdict, maintaining strict confidentiality (CC)
  • Completes enrollment/intake on eligible clients
  • Provides one hour of crisis services per day to all crisis stabilization clients
  • Facilitates daily crisis stabilization individual sessions and/or groups
  • Lead the Bridges team in crisis stabilizations services to meet basic crisis stabilization needs of clients; including assistance with developing and revising an ongoing safety plan, stabilization plan, and connecting to medical, therapeutic, or other providers as identified (CC)
  • Assist program supervisors with training Bridges staff
  • Schedule client health screenings and health examinations, as applicable
  • Assists clients in the identification of strengths, needs and goals to develop an individualized service plan (TL)
  • Documents client activity, service plan progress and needs per established record keeping procedures
  • Provides problem solving and crisis intervention services to empower clients to develop self-sufficiency
  • Screens for mental health and substance abuse disorders and provide linkages to appropriate services
  • Assists clients in locating, obtaining, and retaining suitable housing though housing search and placement services (TL)
  • Ensure required information is collected, entered in prescribed formats, and reported in a timely manner
  • Monitors client self-administration of medication and follows procedures of recording
  • Willingness and ability to obtain/maintain certification to administer non-parenteral medications in the state of Iowa
  • Accurately record client referrals and service outcomes on designated databases
  • Submits prior authorization forms to client managed care organizations to secure funding for CSRS services
  • Psycho-educate clients on life, social and coping skills
  • Demonstrates knowledge of current community and HFS programs and resources to meet client needs, including providing connections to mainstream benefits, income support services and Integrated Health Home services
  • Utilizes trauma-informed care, person centered and culturally competent principles in providing services to clients
  • Evaluates efficiency and effectiveness of community service providers on an ongoing basis to ensure clients are receiving quality care
  • Orients clients to Bridges protocols and addresses client non-compliance as necessary to create a safe and secure environment for all residents and staff
  • Conducts client room checks and searches per program protocol
  • Always displays a courteous and caring attitude to the clientele, volunteers, and visitors of the Agency
  • Abides by all specific program and Agency procedures, policies, and requirements
  • Cooperates and collaborates with program area staff, volunteers, and other Agency staff
  • Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media
  • Strives to make connections between the agency and the larger community whenever possible to contribute to the agency’s ongoing fundraising and friend-raising efforts.
  • Essential functions of this job are to be performed on company physical work site
  • *Is available by telephone for on-call emergencies on a rotating basis
  • Performs other program related duties as assigned
